Summary: Five factors cause massive project failure. Wrong initiatives contradict business logic and produce outcomes that few people want, bring little value, or undershoot advantages because they are difficult to implement. A second factor is unreasonable limits, such as insufficient cash, personnel, time, or other inputs.
